Maternal and Child Visitation Services

Maternal and Child Visitation Services

Supporting families from pregnancy through early childhood with in-home nursing visits and care coordination.

Home services support families from pregnancy through childhood to promote healthy child development.

Childhood Connection Visits

The Weber-Morgan Health Department offers home visitation services designed to support the health and development of children and families. Visiting nurses provide guidance, education, and connections to community resources.

Home visiting nurses work with families during pregnancy and throughout early childhood to ensure children grow and develop in a healthy, supportive enviornment.

Home Visitation Programs educate families on the following topics and may provide services such as:

  • Tracking and understanding child developmental milestones
  • Immunization information and resources
  • Child health and safety education
  • Parenting education and support
  • Maternal health education
  • Postpartum depression screenings
  • Coordination with health services and community resources
  • Early intervention services when needed

All families living in Weber and Morgan Counties qualify for home visitation services starting as early as pregnancy to age nine, regardless of insurance coverage or family income. Services may be provided in your home or at the Weber-Morgan Health Department, depending on what works best for your family.

All services are confidential and provided at no cost.

Developmental Screening

Families interested in home visitation services should register for and complete the Ages and Stages Questionnaire (ASQ) (for Spanish, please visit Ages and Stages Questionnaire – Spanish) to measure your child’s developmental milestones up to age six. This screening helps identify potential delays early so families can receive early support and services as needed. Families that complete the Ages and Stages Questionnaire may receive a $25 gift card (while supplies last).

Home visitation services are also available for pregnant individuals, families with children outside the ASQ age range (ages 6-8), and families seeking education, support, or connections to community resources. Contact Clinical Nursing Services below and ask how you can be supported through our Home Visitation Services.

📞 Have questions about eligibility, scheduling, or want to refer a family for home visitation services? Call (801) 399-7250 and ask for a home visiting nurse, or find more information about the programs in English (PDF, 461 KB) or Spanish (PDF, 366 KB).
