Immigration Services

Two appointments are required to complete an Immigration Physical at Weber Morgan Health Department.

First Appointment

Lab work for tuberculosis (TB), syphilis, and gonorrhea screening
- No fasting is required for the lab work
Health history information collected
Required immunizations for immigration physicals
- Immunization fees not included in Immigration Physical exam fee

Second Appointment

Physical exam with a Civil Surgeon
- A Civil Surgeon is a licensed doctor who is certified to provide immigration physicals in the United States
Completed I-693 (PDF,680 KB) form copied and sealed in envelope
- Envelope and additional copy given to patient

Complete Immigration Exam – All Ages	$340
Rescheduling/No-Show Fee	$75

Complete Immigration Exam does not include immunization or radiology fees, if needed.

Weber-Morgan Health Department is unable to bill insurance for immigration exams. Some insurance plans may cover required vaccines.

Payment is due at the time of the first appointment and is non-refundable.
