Supporting families from pregnancy through early childhood with in-home nursing visits and care coordination.
Available Programs:
Home Visitations Programs
We offer the following home visitation services: Targeted Case Management, Preschool Development Grant, and Prenatal to 5.
Targeted Case Management
Targeted Case Management (TCM) provides in-home nursing support for families enrolled in Medicaid.
A registered nurse visits families in their home to monitor developmental milestone, support parents and caregivers, and connect families with medical, social, educational, and other support resources.
Preschool Development Grant
The Preschool Development Grant (PDG) provides no cost services to families in Weber and Morgan Counties starting during pregnancy to age 9, regardless of insurance coverage or family income. A registered nurse helps families monitor developmental milestones, answer parenting questions, screen for postpartum depression, gain access to resources and specialized care, and more – all with the goal to help improve child health outcomes and coordinate care in a family’s community. Services are provided in our office or your home confidentially, based on what works best for your needs.

Prenatal to 5
Prenatal to 5 is a referral-based service for children from birth to age five. A registered nurse visits families in their home to assess health needs, provide education and parenting support, identify medical or social risk factors, and coordinate services with providers and community agencies.

Baby Your Baby Program
The Baby Your Baby Program supports healthy pregnancies by providing education and encouraging early prenatal healthcare usage before the 13th week of pregnancy. Eligible individuals may receive prenatal insurance coverage while waiting for Medicaid approval.
Baby Your Baby services may include:
- Prenatal lab test
- Ultrasound exams
- Prenatal visits with your doctor
- Prenatal vitamins
- Emergency room visits related to your pregnancy
- Dental services

Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Program
Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) is a part of the Utah State Medicaid Program and provides medical and dental care for children ages 0-21 who qualify for Medicaid. Through regular checkups, Medicaid-eligible children stay healthy, meet developmental milestones, and receive necessary follow-up care.
Weber-Morgan Health Department participates in the EPSDT Program by providing reminder calls to eligible families so they can take advantage of their available benefits.
Benefits may include preventative physical exams, hearing and vision screenings, mental health services, recommended immunizations, dental exams and associated care.*
If additional medical concerns are identified, EPSDT works with the doctor to ensure appropriate and necessary care is provided.

*Dental care may include two exams per year, x-rays, sealants, fluoride treatments, and necessary restoration work.
